Lost Certificate: Applying for re-issue
You are issued a matriculation (matric) certificate when you pass Grade 12. A matric certificate may be re-issued to people whose matric certificates have been lost, stolen or damaged or to people who have changed their surnames.
Steps to follow
- Apply in person at Assessment and Examinations Office in King William’s Town.
- Bring originally certified copy of your Identity Document (ID)/Birth Certificate. You must also bring a R48 for Postal Order and a R15 for the confirmation statement made out to the ECDoE has to accompany the application form.
- Provide details of where and when you wrote your examination, including the following:
- Year
- District
- School or examination centre
- Your examination number

It may take 8 to 10 weeks.
